It is a sequel to Drishyam which was released in 2015 and Drishyam 2, which released in 2022. Ajay Devgn returns as the central character, with Jaideep Ahlawat, Tabu, Shriya Saran, Rajat Kapoor, Prakash Raj, Ishita Dutta and Mrunal Jadhav in key roles.

According to him, both films begin from the point where Drishyam 2 ended, but the Malayalam version is "more of a family drama".

Pathak also said the makers liked the Malayalam story but recognised that the Hindi audience has a different "taste and palate". He pointed out that the team had made changes to Drishyam 2 as well, and audiences had appreciated those differences. For the third film, he confirmed that changes have again been made to the story and screenplay, adding that viewers will notice the difference.
