According to the case report published in The Lancet by the doctors who treated her, Bågenholm found a small pocket of air under the ice and stayed conscious there for 40 minutes before her heart stopped from the extreme cold.
Eighty minutes trapped in freezing water
Falkenberg and Næsheim tried to free her but couldn't break through the ice. They called for help on a mobile phone and held onto her skis so the current wouldn't sweep her away. A rescue team skied down as fast as it could, but its snow shovel couldn't cut through the ice either. A second team arrived from below with a pointed gardening shovel, and that finally worked. According to CNN, rescue leader Ketil Singstad later admitted he thought they were pulling out a dead friend.
By the time she was freed, Bågenholm had been underwater for 80 minutes. She had no pulse, her blood had stopped circulating, and her pupils were fully dilated. CBS News reported that her friends, both doctors themselves, started CPR immediately and kept it going through the hour-long helicopter flight to Tromsø.

When Bågenholm reached Tromsø University Hospital, her core body temperature had dropped to 13.7 degrees Celsius, or 56.7 degrees Fahrenheit. At the time, this was the lowest body temperature anyone had ever survived after accidental hypothermia. According to BBC News, the previous record was 14.4 degrees Celsius, recorded in a child.
Dr. Mads Gilbert, the anesthesiologist leading her care, later described her condition bluntly. notes that he recalled her skin as ice cold, her pupils fully dilated, and the heart monitor showing no activity at all. Even so, his team refused to call time of death. Their approach, in simple terms, was to warm a patient up fully before ever assuming the worst. More than a hundred doctors and nurses worked in shifts for nine hours to save her, connecting her to a heart-lung bypass machine that slowly warmed her blood outside her body before returning it to her veins. Anna Bågenholm's heart stopped at about 19:40; it did not begin beating on its own again until 22:15, approximately two hours and 35 minutes later.

The same cold that nearly killed Bågenholm likely also protected her brain. When body temperature drops this low, the metabolism slows sharply, and organs need far less oxygen to survive. According to CNN, doctors explained that a very cold brain needs only a fraction of the oxygen it does at a normal 98.6 degrees Fahrenheit, which may explain how her brain avoided permanent damage despite going without a normal heartbeat for so long.
Ten days of silence, then a slow comeback
Bågenholm did not wake up until ten days after the accident, and when she did, she was paralyzed from the neck down. She has said she was initially angry with her colleagues for saving her, fearing a life without dignity, and later apologized once she began to recover. Her kidneys and digestive system were also not working properly, so she spent two more months in intensive care before continuing her recovery.

A case that changed emergency medicine
Bågenholm's survival did more than make headlines. It reshaped how doctors around the world treat severe hypothermia and cardiac arrest. Her case is still cited in medical textbooks and journals for one central lesson: in extreme cold, a patient without a pulse may still be saved. BBC News reported that Gilbert later said that even deep hypothermia victims without a heartbeat should be treated as potentially recoverable, as long as bystanders act fast with CPR, rescue teams respond quickly, and the medical team simply doesn't give up too soon.
Today, decades later, Bågenholm still has minor nerve-related symptoms in her hands and feet, but has otherwise made a near-complete recovery. What began as a terrifying accident on an icy Norwegian mountainside became one of the most studied survival stories in modern medicine.
