A daily prebiotic fiber supplement found in many fruits and vegetables may help reduce knee pain and improve physical function in people with osteoarthritis, a new study finds.

If you or someone in your family has been struggling with knee osteoarthritis, here's a bit of good news that doesn't involve another round of painkillers. Scientists at the University of Nottingham have found that a fibre called inulin, the same one naturally present in onions, garlic, bananas, chicory root and Jerusalem artichokes, may meaningfully ease knee pain and improve day-to-day movement.

The findings come from the INSPIRE trial, published in the journal Nutrients, and they're already being called an "exciting" development by the researchers involved.

Why This Natural Fiber Matters for Millions

Knee osteoarthritis is one of those conditions that quietly limits life for a huge number of people, particularly as they get older. Climbing stairs becomes a chore, morning stiffness lingers, and everyday walks start to feel like a task rather than a pleasure. Doctors typically manage it through painkillers, which come with their own baggage of side effects, or through structured exercise programmes that many patients simply find hard to stick with over weeks and months.

That last part turned out to be one of the more striking results of this study.

What the Researchers Actually Did

Over six weeks, 117 adults living with knee osteoarthritis were split into four groups. One group took inulin supplements, another followed a digital physiotherapy exercise programme, a third combined both, and the last received a placebo.

Both inulin and the exercise programme brought down knee pain on their own. But here's where it gets interesting, only the fibre group saw improvements in grip strength and a drop in pain sensitivity, markers that hint at changes in how the nervous system is actually processing pain signals, not just masking them.

And then there's the dropout numbers, which tell their own story. Barely 3.6% of people in the inulin group left the trial before it ended. Compare that to 21% among those doing the physiotherapy programme, nearly six times higher. Popping a supplement into your daily meal, it seems, is a lot easier to keep up than committing to structured exercise, however beneficial that exercise might be.

The Gut-Pain Connection Explained

So how does a fibre found in vegetables end up affecting your knees? The answer lies in your gut.

Inulin is what's known as a prebiotic, essentially food for the trillions of good bacteria living in your digestive system. When these bacteria break it down, they produce short-chain fatty acids, and one of these, butyrate, plays a role in dialling down inflammation and influencing pain pathways throughout the body.

Researchers also observed higher levels of a gut hormone called GLP-1 among those taking inulin, which appeared linked to stronger grip strength. That's led scientists to suspect there may be a broader "gut-muscle-pain" connection worth exploring further, one that could stretch beyond arthritis into how our bodies age and stay physically resilient overall.

What the Reserchers Are Saying

Dr Afroditi Kouraki, who led the study at Nottingham's School of Medicine, called it a genuinely hopeful sign for patients. In her words, something as simple as stirring a fibre supplement into breakfast or yogurt could translate into real, measurable relief from pain and improved movement, without the burden that comes with sticking to an exercise regime long-term.

Professor Ana Valdes, the study's senior author, pointed to the GLP-1 and grip-strength link as one of the more intriguing threads to pull on next, suggesting it could reshape how scientists think about ageing and muscle health, not just arthritis.

Reacting to the findings, Professor Lucy Donaldson of Arthritis UK noted how deeply arthritis pain can erode quality of life, with recent surveys showing a majority of sufferers dealing with pain most or all of the time. She welcomed the research as a reminder that both diet and physical activity have distinct, complementary roles to play in managing the condition.

To be clear, this isn't a cure, and nobody's suggesting you toss out your knee medication tomorrow. But for a condition that affects such a large share of the ageing population and often comes with limited, imperfect treatment options, a cheap, easily available dietary fibre showing this kind of promise, with almost no one dropping out of the trial, is worth paying attention to.

As always with early-stage research, larger and longer trials will be needed before doctors start recommending inulin supplements as standard practice. But for now, it's a fairly compelling reason to make sure your plate has a bit more garlic, onion, or banana on it.
