Faridabad man applies for father's death certificate, receives birth certificate; clerk penalised
The Haryana Right to Service Commission penalised a clerk in Faridabad for mistakenly issuing a birth certificate instead of a death certificate, fining him Rs 3,000 and ordering Rs 3,000 compensation to the applicant.

According to the commission, the applicant had applied for his father’s death certificate on March 22, 2025. His father had passed away on March 19, 2025. However, the system generated a birth certificate of a child, which was uploaded and the application was marked as completed.
A spokesperson of the commission said, “During investigation, it was found that the certificate issuance process was handled by a clerk at the NIT Zone-II office of the Municipal Corporation Faridabad, who uploaded the wrong certificate and closed the application.”
Fine imposed on erring clerk
The RTS Commission imposed a fine of Rs 3,000 on the clerk under Section 17(1)(h) of the Haryana Right to Service Act, 2014. Additionally, the commission directed a compensation of Rs 3,000 to be paid to the applicant. The total amount of Rs 6,000 will be recovered from the concerned employee’s salary.Tehsildar penalised for delay in marriage certificate
In another case, the commission imposed a fine on the tehsildar-cum-marriage registrar of Hansi for a delay in issuing a marriage certificate. The commission said there was a four-month delay in processing the request.The registrar had cited the absence of a birth certificate as the reason for the delay.
The commission imposed a fine of Rs 1,000 on the official and directed him to pay Rs 5,000 as compensation to the applicant.
