FDA Panel votes in favor of a $949 Cancer detecting blood test, but is Galleri ready for approval?

A single blood draw that can search for more than 50 cancers sounds like a medical breakthrough. Galleri is now one step closer to formal approval in the United States, as per a report by USA Today.

An advisory panel to the Food and Drug Administration voted in favor of the test on September 23. The decision gave GRAIL, Galleri’s developer, an important regulatory victory.

Debate around the cancer-detecting blood test is far from settled, though. A large clinical trial failed to achieve its main objective earlier this year. Galleri can also miss cancers or produce a signal that further testing does not confirm.

What did the FDA panel decide?

Ten outside experts reviewed Galleri’s use in adults aged 50 or older. Seven panel members said its benefits outweighed its risks. Two disagreed, while one abstained. Six members found the test effective enough to support approval. Four voted against it. All 10 members backed its safety.

The FDA is not required to follow the recommendation. A final decision is expected in the coming months.

Approval would make Galleri the first FDA authorized multi-cancer early detection test. The product is already available by prescription as a laboratory developed test.

How does the cancer-detecting blood test work?

Cancer cells can release small DNA fragments into the bloodstream. According to the USA Today report, Galleri examines chemical patterns on those fragments. A machine-learning system then looks for signals associated with cancer.

When a signal is detected, the test predicts where it may have started. The result gives doctors a possible place to begin further investigation.

Galleri does not diagnose cancer. A positive result must be followed by established diagnostic procedures. Imaging or a biopsy may be needed. A negative result does not rule out cancer either. GRAIL says patients should continue routine screening recommended by their doctors.

Why is Galleri controversial?

Supporters believe Galleri could help find cancers that lack routine screening programs. Earlier detection may give some patients more treatment options.

Critics say the evidence has not yet proved that widespread testing will save lives. Questions also remain about whether Galleri consistently catches cancer at an early stage.

A trial involving more than 142,000 NHS participants tested Galleri alongside standard screening. The study did not produce a statistically significant reduction in combined stage 3 and stage 4 cancer diagnoses. Its main endpoint was therefore not met.

Some secondary findings were more encouraging. Later screening rounds showed a reduction in stage 4 diagnoses for certain serious cancers. Adding Galleri also increased the total number of cancers detected.

Those results did not erase the failed primary endpoint. Longer follow-up will be needed to determine whether testing reduces cancer deaths.

What happens after a positive Galleri result?

A “cancer signal detected” result starts another medical process. It does not provide a final answer. Doctors may order scans based on the predicted organ of origin. A biopsy may follow if imaging finds something suspicious.

No cancer may be found after the diagnostic workup. Such an outcome can bring anxiety and extra medical costs. Galleri can also detect slow-growing cancers that may never become life-threatening. Finding them could expose some patients to unnecessary monitoring.
