OceanGate CEO ignored flawed Titan sub before deadly Titanic trip, says US Coast Guard report, reveals reason behind disaster

The U.S. Coast Guard on Tuesday said operator failures were the primary cause of the Titan submersible disaster, which killed all five onboard during a June 2023 dive to the Titanic wreck.

OceanGate CEO Stockton Rush, who died on the deep-sea submersible alongside four others, ignored critical data leading up to the vessel’s implosion, according the U.S. Coast Guard.

The Coast Guard's report said that the implosion was "preventable."


The 335-page report, released Tuesday, revealed that OceanGate had “critically flawed” safety practices and a toxic workplace culture — and that Rush’s “negligence” contributed to the deaths of those on board.

Rush, who acted as the Titan sub’s pilot, “exhibited negligence that contributed to the deaths of four individuals,” the report found. It also “identified evidence of a potential criminal offense.” Had Rush survived, the Coast Guard would have recommended that the Department of Justice consider pursuing a separate criminal investigation.

The Coast Guard convened its highest level of investigation in the aftermath of the 2023 implosion off Canada. The disappearance of the Titan led to a search that garbed worldwide attention.

There were no survivors. The Titan was owned by OceanGate, a private company based in Washington state. The operator of the submersible, OceanGate head Stockton Rush, was among the five on board who died.

The submersible disaster has led to lawsuits and calls for tighter regulation of the developing private deep sea expedition industry.
